All information given is true and correct No information that could have a material bearing on this application to host an international student has been omitted or intentionally withheld. It is fully understood that making a false statement is an offence. The applicant will only be able to host an underage student once they have passed our Police check procedure. If an applicant's home does not pass our house inspection procedure, it will not be accepted for hosting one of our international students. This inspection covers cleanliness, hygiene, tidiness, safety and security for the student, as well as adequate room furnishings and equipment. By filling out the information on this form, the applicant has agreed to the disclosure and use of the information on this form by New Zealand Care to authorized bodies in response to requests under statutory authority. The applicant has the right to have access to all personal information held by New Zealand Care, and a right to ask for anything inaccurate to be corrected. The applicant has an obligation to inform New Zealand Care if/when any of the information changes.
